A 53-year-old man with bilateral periorbital congestion, erythema, and proptosis presented with the symptoms of Erdheim-Chester disease. Hertel exophthalmometry measured exophthalmos of 27 mm and MRI of the orbit revealed bilateral intraconal masses with increased signal on T2-weighted images. Long bone radiographs revealed fibrous sclerosis in the distal left fibula. A computed tomography scan demonstrated retroperitoneal fibrosis and a biopsy confirmed the diagnosis of Erdheim-Chester disease. Oren Zarif
